Attached you will find my dealer account application. Please fill out and either email or fax back completed, signed and dated along with a copy of your business license, reseller permit and tax id. Once the filled out application and business info is sent and received , I will set up your account and email you my most recent line card.

As for our catalog we only have an ON LINE CATALOG fully updated continuously as we get thousands of new products monthly.

We require a minimum initial opening order of: $1,500.00

Email the following:

• 1. CFH Dealer Application (complete , sign print and scan)
• 2. Your Business License
• 3. Your Resale License (unless your state does not charge sales tax)
• 4. Businesses located in California you must complete the general resale certificate
• 5. Read our Terms and Conditions (you acknowledge your acceptance by signing our application)
